Centene Corporation (NYSE: CNC) is the largest Medicaid managed-care insurer in the United States, and simultaneously the largest ACA Marketplace (Obamacare) insurer as well. It is not a household name the way UnitedHealth is, but by membership count Centene is a giant — operating as a "managed care organization" (MCO) coordinating healthcare networks for lower-income and government-subsidized populations.
Centene's business is split into four segments:
To understand Centene today, you need to understand what happened over the past year. On 1 July 2025, Centene withdrew its FY2025 guidance after industry data showed weaker-than-expected ACA Marketplace enrollment growth and a sicker enrollee mix (higher morbidity) across key states. The market reaction was brutal — the stock fell -40.4% in a single day, one of the largest single-day drops for an S&P 500 company in recent memory. FY2025 adjusted EPS ultimately came in at just $1.75, a massive miss against the original April 2025 guidance of $7.25.
Following the crash, CEO Sarah London moved aggressively — cost cuts (including voluntary buyout offers across roughly 61,000 employees), Marketplace product repricing, and Medicaid rate renegotiations with states. Results started showing up in 2026: Q2 2026 posted adjusted EPS of $2.51 versus a consensus estimate of just $1.10 — a large beat. The Health Benefits Ratio (HBR, the ratio of medical costs to premiums) also improved from 93.0% (Q2 2025) to 89.6% (Q2 2026), a sign margins are recovering.
One notable confidence signal: CEO Sarah London personally bought roughly 19,230 shares (~$490,000) with her own money over the past 6 months — insider buying of this kind is typically viewed positively because management is putting its own money where its mouth is.
Centene is NOT Shariah-compliant (Non-Compliant) per the Zoya US stock reference. This is not company-specific bad news — it is the standard status for nearly all health and general (P&C) insurance companies globally, including peers like UnitedHealth, Elevance, and Molina. The reasons are:
Bottom line: Centene is an educational case study for Mahersaham students to understand US healthcare-insurance industry dynamics — not an investment recommendation, given its Non-Compliant status.
